Medical Unit I

Medical Unit I

Medical Unit-I is a 60 bedded floor, it has a semi ICU room with eight beds which looks after critical patients. Children are admitted mainly with general pediatric problems which includes Diarrhea, Malnutrition, Pneumonia, Tuberculosis, Asthma, Chronic Liver Disease, diabetes mellitus and hematological disorders. The unit has twice a week and third Sunday as emergency days admitting all Paediatric emergencies like fever with fits, status asthmatics, septicemia, and meningitis, renal failure etc. There are two out patients’ clinics every week which deal with ambulatory Paediatric care.

The department is responsible for teaching and training of a number of under graduates from Sindh Medical College as well as post graduate’s students of DCH, Membership and Fellowship of College of Physicians & Surgeons of Pakistan.

There are 60 beds admitting children mainly from general pediatric including common pediatric illness like Diarrhea, Malnutrition pneumonia, Tuberculosis, Asthma, Chronic liver disease etc.  There is a special unit from looking after children with Diabetes Mellitus who are newly diagnosed or admitted from our diabetes clinic for poor control. There are also admissions from endocrine clinic on a day care basis for dynamic endocrine investigations, such as insulin tolerance test, Water deprivation test etc.

The unit also has twice a week emergency days admitting all pediatric emergencies like fever with fits, status asthmatics, septicemia, meningitis, renal failure etc. There are two outpatient clinics every week which deals with ambulatory pediatric care. There are also designated outpatient clinic for pediatric endocrinology and pediatric Diabetes.

The department is responsible for teaching and training of a number of undergraduates from Sindh Medical College as well as post graduate’s students of DCH, Membership and fellowship of college of physicians & surgeons Pakistan.

Unit-III is a 45 bedded ward which includes a Diarrhea Treatment Unit and a Therapeutic Feeding Centre for severely malnourished children. There is also an Intensive Care Unit for very ill children. Other beds are for general pediatric patients.

The job was full time residential. It included training in procedures like vene puncture, lumbar puncture, ascitic and pleural tap.

Neonatology Unit

The neonatal Unit of NICH is at present the largest unit in the country. This came as a result of up gradation of old 28 bedded neonatal unit in 2002, budgeted by government and NGOs. Present unit is a 65 bedded unit having NICU with 40 incubators including 6 ventilated beds and a step-down unit with 25 cots. In summer months bed strength reaches around 90. the unit being part of a public hospital , caters round the clock admissions from all over Karachi and adjoinining districts as well as referrals from entire province. The unit provides pre and post care for neonatal surgical cases.

To meet the complex need of poor children affected by cancer, a Pediatric Oncology Unit was established at NICH in 1999 by an NGO Child Aid Association and since then it has been providing free of cost comprehensive care to cancer patients.

The unit has an in patient 8 bedded general ward and 4 isolation cubicles for high dependency patients. The daycare chemotherapy unit provides services six days a week form 9am to 5pm. Outpatient department is also work for six days a week with average daily attendance of forty patients.
There has been a steady increase in number of new registered cases during previous years and till date over 2800 patients are registered for evaluation and chemotherapy. We receive patients from all over Pakistan and around 55% patients came from outside Karachi. Half of the registered patients had leukemia (Blood Cancer), a disease which requires log duration of chemotherapy regular blood transfusion during initial intensive phase of treatment.

The unit has its own laboratory which provides free of cost facilities including Immunophenotyping and Histopathology tests. The Cytogenetic and Molecular Labs established at NICH by CCA also free sophisticated diagnostic facilities including Karyotyping, FISH and PCR.

Nephrology & Dialysis Unit

Nephrology division of Department of Pediatrics has been working since 1988 on third floor of NICH Main building. With the efforts of Prof Afroze Ramzan, Pediatric Nephrologist, Head of department of Pediatrics and director NICH, new nephrology and dialysis unit has been developed and become functiong from December1, 2006.

Bed strength: 20 beds
Hemodialysis Stations:  8
Acute PD beds   8
Night Cycler Peritoneal Dialysis Machines: 2

  1. Acute Manual Peritoneal Dialysis: 24 hour free peritoneal dialysis for children of 1 month -14 years
  2. Hemodialysis: Acute Hemodialysis facility for 24 hour free of cost
  3. Long-term maintenance hemodialysis:  Available to all children who need dialysis with a capacity of seven New HD machines and we can dialyze 6-7 patients in morning and evening shifts. New state of art   Reverse Osmosis system for Water Purification necessary for HD and dialyser Reprocessor for reuse of dialyzers has been functiong.
  4. CAPD has been recently started in patients waiting for transplant and can not   do hemodialysis or those who are too young to do hemodialysis
  5. In-Center acute CCPD: Automated PD is provided to children with ESRD having decided for transplant for short period (4-8 wks) to train the parents about home dialysis(CAPD) which will be continued till renal transplant.
  6. Renal Biopsy: Percutaneous Ultrasound guided kidney biopsy using Monopty Automated Biopsy gun is done   for children with refractory primary nephrotic syndrome, congenital nephrotic syndrome, secondary nephrotics and  all types of glomerulonephritis.
  7. Multidisciplinary services including investigations, work-up and management under one roof of all congenital and acquired renal diseases including various types of surgical procedures, radio imaging (IVU, MCUG, U/S, Color Doppler and  MRI) in collaboration with surgical team.

In physiotherapy, services offered include paediatric rehabilitation musculoskeletal rehabilitation, general orthopaedic conditions neuro-rehabilitation, hydrotherapy, gart training, treatment of pain through Trancutaneous Electrical Nerve Stimulator (TENS) and other eclectrotherapentic modalities. All services are being provided free of cost.

Pediatric Intensive Care Unit

Intensive care unit of NICH is a 13 bedded unit, having 9 neonates and all the patients who are either critically ill or need meticulous monitoring are admitted here. Patients like diabetes ketocidosis, acute severe asthma,GBS, Steven Johnson Syndrome, Cardiac Failure, tetanus are taken on priority basis.

 

Emergency Department

Emergency Department of NICH is 16 bedded and is a very busy place where more than 300 patients are seen daily. 4 beds are reserved for neonates, 6 for those who require intense monitoring treatment and 6 for daycare. A triage committee is present of filter the patients and are admitted accordingly.
